Thank you for choosing Reliable Canine! Please complete the form below with as much detail as possible before your dog’s stay. This information helps us understand your dog’s needs, personality, routines, and preferences so we can provide a safe, comfortable, and enjoyable stay.
Please note: Full and accurate disclosure of any medical or behavioural concerns is required before boarding. If you have any questions or concerns about your dog’s stay, please contact us before submitting the form.
Health & Vaccinations
Dogs must be healthy and free from contagious illness at the time of boarding.
Dogs must be up to date on required vaccinations.
Owners must disclose any medical conditions, allergies, medications, or health concerns before boarding.
Dogs showing signs of illness may be refused boarding or require veterinary clearance.
Behaviour & Safety
Owners must disclose any history of aggression, biting, resource guarding, dog fights, or other significant behavioural concerns.
Dogs may be introduced to our resident dogs only when deemed appropriate and safe based on temperament, behaviour, and compatibility.
Reliable Canine may refuse or end a boarding stay if a dog's behaviour presents a safety concern.
Emergency Veterinary Care
In the event of an emergency, Reliable Canine will make reasonable efforts to contact the owner and emergency contact.
If veterinary care is necessary and the owner cannot be reached, the owner authorizes Reliable Canine to seek veterinary care.
All veterinary expenses are the responsibility of the owner.
Food & Medication
Owners are responsible for providing sufficient food and medication for the entire stay.
All medications must be clearly labelled with the dog's name and instructions.
Owners are responsible for providing sufficient food for the duration of their dog’s stay. If additional food is required, Reliable Canine will purchase it as needed, and the cost will be invoiced separately to the owner.
Drop-Off & Pick-Up
Dogs must be dropped off and picked up during the agreed-upon times.
Owners should notify Reliable Canine if they are running late.
Additional boarding fees may apply for late pick-ups or extended stays.
If an owner fails to pick up their dog and cannot be reached, Reliable Canine will attempt to contact the owner and emergency contact.
In the event of suspected abandonment, Reliable Canine reserves the right to contact the appropriate animal welfare authorities or take other steps permitted by law. Any additional costs incurred for the dog’s care will remain the owner’s responsibility.
Belongings
Owners are responsible for providing all necessary food, medication, and any specific items your dog requires during their stay. Reliable Canine will provide bowls, crates, and bedding.
Reliable Canine is not responsible for lost or damaged personal belongings.
Cancellation Policy
Cancellations made within 14 days of the stay are non-refundable.
No-shows or early pick-ups are non-refundable.
